Showcase New York State Diabetes Campaign

Forum One Communications was selected by the New York State Health Foundation to develop the New York State Diabetes Campaign on Drupal.

About

More than 1.8 million New York State residents have diabetes, and diabetes prevalence has doubled in the last decade. Diabetes also contributes substantially to rising health care costs. The total cost of diabetes for New York in 2006 was estimated at $12.9 billion, including $8.7 billion in excess medical costs attributed to diabetes and $4.2 billion in lost productivity.

In response, the New York State Health Foundation launched a five-year, $35 million campaign in May 2008 to reverse the diabetes epidemic in the State. The Campaign will work to:

Improve clinical care and patient outcomes

The Campaign will support the statewide adoption of clinical best practices at community health centers, hospital outpatient departments, and private practices to improve the access to and quality of primary care for patients with diabetes. This work will ensure that people with diabetes receive the right care, at the right time, in the right setting, and at the right cost to achieve the best health outcomes.

Mobilize communities

The Campaign will support community-based strategies to identify New Yorkers with diabetes who do not know they have the disease, support people in managing their diabetes, get people into clinical care, and promote healthy eating and physical activity.

Promote policy

Under the New York State Health Foundation’s Diabetes Policy Center, the Campaign will support private and public payment policies that enable providers to deliver more comprehensive, coordinated care for patients with diabetes. The Center also will support community-based policy strategies, such as making nutritious foods and recreation spaces more available.

In order to reverse New York’s diabetes epidemic, we need a comprehensive and coordinated response. Therefore, the Campaign will work with partners throughout New York State to spread what has been shown to improve the outcomes and lives of people with or at risk for diabetes.

Rather than being directed at consumers, this Campaign works with the people and organizations that can best help New Yorkers with diabetes prevent and manage diabetes and its complications.

Dr. Neil Calman, the President and CEO of the Institute for Family Health, leads the Campaign in partnership with NYSHealth.
